HOW DO WE COLLECT YOUR PERSONAL INFORMATION?
Where reasonable and practical we will collect your personal information only directly from you. If for some good reason, we are unable to do so, we may involve another organisation. For example, we may need to request a credit report from a credit reporting agency when you apply for a credit facility. Before doing so or prior to collecting the information we need, we will inform you of the type of organisations we intend to disclose your personal information to.

TO WHOM WILL WE DISCLOSE YOUR PERSONAL INFORMATION?
Where possible, we will inform you, at or before the time of collecting your personal information, the types of organisations that we intend to disclose your personal information to.

We will provide you with access to the personal information we hold about you. You may request access to any of the personal information we hold about you at any time. To access personal information that we hold about you, please contact us either by phone or in writing. We may charge a nominal fee for our costs in retrieving and supplying the information to you.

There may be situations where we are not required to provide you with access to your personal information. For example, such a situation would be information relating to an existing or anticipated legal proceeding with you, or if your request is vexatious. An explanation will be provided to you, if we deny you access to your personal information we hold.

HOW LONG DO WE KEEP INFORMATION ABOUT YOU?
By law, we are required to hold certain information about you, even after you are no longer an Intellichoice client. However, after such time you can be assured we will destroy your personal information thoroughly and safely.

We may also hold certain information about you if it is needed for a purpose which you are aware of or reasonably expect us to hold the information. When that information is no longer needed, it will be destroyed or permanently de-identified.

BUSINESS WITHOUT IDENTIFYING YOU
In most circumstances it will be necessary for us to identify you in order to successfully do business with you, however, where, it is lawful and practicable to do so, we will offer you the opportunity of doing business with us, without providing us with personal information.

Such a situation would be where you make general inquiries about interest rates or current promotional offers. Where sensitive information is collected, stored, used or transferred about you, we will only use this information if you have consented or in the exercise or defence of a legal claim. Sensitive information is information relating to your racial or ethnic origin, political persuasion, memberships in trade or professional association or trade unions, sexual preferences, criminal record, or health.

HOW SAFE AND SECURE IS YOUR PERSONAL INFORMATION THAT WE HOLD?
Please be assured that we will take reasonable steps to protect your personal information by storing it in a secure environment. We will ensure that the information is securely protected from misuse, loss and unauthorised access, modification or disclosure. When the information is no longer needed for any purpose for which the information may be used or disclosed, it will be destroyed or permanently de-identified.

IF YOU HAVE A COMPLAINT
We are committed to providing you with the best possible level of advice and service. As part of that commitment, we are focused upon maintaining high levels of client satisfaction by providing efficient and accessible systems for resolving any client complaint.

If you have any complaint about the service provided to you, you should take the following steps:

1. Contact Intellichoice and inform your consultant about your complaint
2. If you do not get a satisfactory outcome after 45 days, you have the right to lodge a complaint to the Financial Industry Complaints Service (FICS). Please download a FICS Complaint Form from www.fics.asn.au and complete it according to the instructions and the information outlined in the document Important Information About Lodging a Complaint. You can contact FICS on 1300 780 808 if you need help completing the form or require any other assistance. The completed FICS Complaint Form and copies of any supporting documents, including details of how the amount of your claim has been calculated can be mailed to:
